Salam,
Political Governmant of Pakistan (and PTA's) exorbitant taxes on international terminating calls are the culprit here.
Political democracy fleecing the citizens for serving their corruption and looting spree is what shall be denounced, instead of incriminating hard-working Pakistanis through cunning laws and regulations.
 
Imposing massive Surtax on International Incomming Traffic (SIIT) is considered worldwide as the dirtiest among such cunning tactics. SIIT in Africa is among the highest in the world and has increased over the last five years, in stark contrast to the developed world where it has decreased significantly, to the point of being eliminated altogether [1]. Pakistani government's imposed SIIT is comparable to that of Senegal, Ghana, Congo, and Gabon [1]. Killing the international telecommunication activity of a country is the surest way of ruining its future, since exorbitant SIIT generates massive negative effects for local consumers, local businesses, MNOs and governments [1]. In particular, the SIIT affects a significant proportion of inbound telecom traffic and generates a domino effect in other telecom-dependant international businesses. This effect, combined with an increased flow of illegal traffic, further reduces demand and service quality and leads to increasing prices and the cost of doing business in the affected countries [1]. In the medium term, the flow on impacts could damage employment opportunities, social cohesion, investment (particularly in communication based business), international competitiveness, terms of trade and government telecom tax revenues [1].
 
As to the misleading IMF loan (and other pseudo-economics) logic:
 
One, if the PPP democracy cared even a bit about paying back IMF and World Bank loans, such colossal loans wouldn't have been taken in the first place. You would be shocked to learn that the former disgraced democracy plundered over USD 12 Billion in loans from IMF in its five year reign [2], the largest ever loans in the history of Pakistan.
 
Two, if the corrupt democracy had any concern for the repayment of loans, the corruption and wastage in government departments would have been atleast looked into. You should be outraged to learn that after over three years of delocracy's rule, in 2011, Pakistan ranked 134 on the transparency international corruption index, out of 174 countries. In 2012, as the democracy was making the final-run, Pakistan's ranking dropped even further from 134 to 139, making Pakistan the 34th most corrupt country in the world, tied with Kenya and Nigeria. [3]. Just 6 months later, as the democracy was stuffing its bags, the World Justice Project in Washington rated Pakistan as 7th most corrupt government in the world [4].
  
Three, if the governmant decides to get rid of loans, it would cut down on giving (selling?) tax exemptions to elites and influential lobbies. We should hold the democracy by the neck and demand: Why were the tax exemptions given to the elite in the past 54 months so enormous that they exceed the loans acquired from the International Monetary Fund to correct the balance of payments [5]? Shocking, isn't it? Meanwhile, PPP government continued to bleed over Rupees 7 Billion every day in corruption [5]. A single day of corruption-less democracy would have given savings comparable to 5 years of these blood-sucking telecom taxes!

Its great that people provide numbers but let's be realistic its not going to help in curbing it.
Its the mobile operator who can actually stop it by utilising very common monitoring facilities.
Identify SIMs from CDR's calling more than 100 unique CLI.
Identify SIM's making call without any delay its humanly not possible to make calls after every 10 seconds the first call ends
Identify the SIM's which are making calls throughout 24 hours.
The problem that we can see over here is that our operators are not looking at the bigger picture, they only look at how much sales they are making irrespective of what the other person is using your SIM for.
Cyber Crime wing of FIA should look into these aspects as well and I'm sure that they will get much more detailed info if they start looking at these things and it will be much easier to locate these set ups in the country.
I'm sitting out of Pakistan and I know how important is the Foriegn currency in flow into the country, specially as Pakistan has to pay a huge amount in next few weeks to IMF as instalment of loan.

Grey telephony is on drastically rise in Pakistan after Ministry of IT and
PTA implemented controversial International Clearing House (ICH) agreement,
under which call rates to Pakistan form overseas were drastically increased
from 01 September 2012.

If you receive a call from overseas and CLI on your cel phone is from a
local Pakistani number, it means that the call is coming from a grey
telephony route.

Cyber Crime Wing of FIA has requested for help in identifying such numbers
so that they can nab people involved in grey traffic.

If you receive a call from overseas and CLI on phone is local, kindly report
the number that appears on your CLI to mailto:amir.nazir%40nr3c.gov.pk.
